Babies have very specific nutritional needs to be able to develop through the first stages of life healthily. They require milk that is high in protein for the essential amino acids to aid growth. The milk must also have a high fat content to support the quick growth rate of babies. They also require a combination of vitamins and minerals; Iron, for growth and healthy blood Zinc, for enzymes and immune function Calcium, for healthy teeth and bones Vitamin D, for calcium and phosphorous absorption to support healthy teeth and bones Vitamin A, for vision. Vitamin C, for healthy connective tissue, antioxidant protection and iron absorption Fluid is also very important for babies as the kidney and other organs are not developed enough at birth to function fully. The Department of Health recommends that babies are breast fed for the first 6 months before any solid foods are slowly and individually introduced. Breast milk is the best nutrition for babies for several reasons. It carries the mother’s antibodies helping the baby’s immunity from disease and infection. The milk will be tailor made for the babies particular nutritional needs and will change according to the baby’s changing needs. It is easier for a baby to digest than any other type of milk. It will be sterile and at the right temperature. At about six months old, when a baby can sit up and hold its head steady, solid food can be slowly introduced in stages in addition to the baby’s milk. The first stage would be to give very soft food for example, pureed fruit or vegetables, baby rice or porridge or plain dhal. Finger food may also be introduced, such as bread crusts or soft fruit. This is more for the baby to practice the motions of eating and to start using the relevant muscles, than for nutritional value. It is important to introduce foods individually in case the child has an allergic reaction to something. This allergen can be identified more easily if it is a new food. CYPOP2-5. 1 The second stage at about 8 months old starts to expand on the variety of foods and textures. It is important to add foods with soft lumps to encourage the use of muscles in the mouth. This is early preparation for speech. Foods may be introduced such as meat, fish and eggs. Harder finger food such as raw vegetables can help with teething and exercise the jaws and chewing muscles. The third stage of weaning at about 10 months children can start to eat the same sort of foods as the rest of the family. Meals can be chopped or minced and finger foods such as toast and biscuits can be given. It is important that children under one year old still continue to have some form of milk in their diet. They also need plenty of other fluid. Water and some cool herbal teas are preferable to juice and squash as they are high in sugar content. As a practitioner involved in a child’s weaning it is important to do so according to the parent’s wishes. They may have cultural or religious practices to be adhered to or may want their child to fed organically or on a vegetarian diet. Some families prefer to let the baby wean themselves. This is known as ‘Baby led weaning’ and is done by giving the child a choice of finger foods and letting them choose what and how to eat. Food and eating can become a psychological issue and difficulty for many children. Parents can become frustrated and concerned about eating habits and children can use food as a tool for getting what they feel they want. There are different types of formula milk for different stages of babies’ development and different types of diets. Babies require different levels of nutrients as they grow. Some babies find digestion of proteins difficult and some families prefer not to give their babies animal products. Most formula milk is based on cow’s milk that has been changed to resemble breast milk more closely. Vitamins and minerals are supplemented and the levels of carbohydrates and proteins adjusted. This type of formula is suitable for most babies, although the proteins in cow’s milk are the most difficult to break down. This difficulty manifests in many children as eczema. Goat’s milk formula may be used as an alternative as the proteins are less complex. Soya bean based formula may be given to babies but is not recommended for those younger than six months old. Children can have severe allergies to soya and may also have difficulty breaking the proteins down if their digestive system is not yet developed enough. Some babies have allergies and intolerances to certain substances from birth. If breast feeding is not possible, or sometimes in addition to breast milk, they may be prescribed a Hydrolised-protein formula. The ethical concerns

The ethical concerns that I have related to this dilemma are many. What is the doctor’s responsibility to try to stop the mother’s contractions? What are the limits of the attempts that should be made to save the child?   Should the mother be allowed to risk her own life to attempt to save the life of a child that is probably not viable outside the womb? Should the doctor plan a cesarean section despite the fact that the infant will probably die as soon as it is removed from the mother’s womb?   I can’t imagine making this decision personally, but many mothers are forced to make it every day. Here is the situation that lead to my ethical quandary.I have a patient who is 3 week ante partum and has had premature rupture of membranes. This condition could cause hemorrhaging for her and death of the infant in uterus. In layman’s terms, both she and the infant are at risk of death. She is starting to contract and the physician will not do anything si nce the fetus is not considered viable. The physician has described the issues of having a vaginal birth versus a cesarean section with this patient because the fetus is breech. The patient wants everything to be done to save this baby. As described above, the issues are exceedingly complex. The physician appears to have determined that the child is a lost cause and is thinking only of the health of the mother, but this is contrary to her wishes. Should the mother’s desire to save her child be allowed to override her own survival instincts? And, what role, if any, should the child’s father have in decision-making process?My literature survey for this situation was amazingly frustrating. I expected there to be a great deal of study materials available regarding this topic. It is, in essence, the quintessential ethical debate: do you save the life of the mother or the life of the child?   And, there is the question of the doctor’s ethics. Should he be able to de termine the best medical course of action if it is contrary to the mother’s wishes? And, who determines when a fetus is viable? Can we allow it to be based on an arbitrary date?I found a lot of older research regarding the ethics of abortion and approaching the discussion of fetal viability from that point of view, but there was nothing recent and nothing than dealt with miscarriages as opposed to abortion. And, there was nothing that talked about the discussion of the life of the mother versus the life of the child. I think this would clearly be a great place for additional study. I think specifically the ethical question of whether medical decisions should be made contrary to the patient’s wishes should also be considered.Right now, as a society, we allow a person to make their own decisions about their health care even though we do not allow them to determine when or how they die.   What I did find were several articles regarding the mental trauma that miscarriage and stillbirth inflict on the mother and an interesting article promoting the development of advanced directives regarding pregnancy health care. Of all the articles, this is the one that I found most interesting and directly applicable to the situation at hand.In this article, Anita Caitlin proposes that obstetricians think outside the box and promote the development of advanced directives for prenatal and delivery care.   The proposal is simple, just as a person can create a living will for care during a terminal illness or traumatic injury, a pregnant woman would in her early weeks of pregnancy discuss in depth with her doctor the potential things that could go wrong and develop a plan of action.   For instance, a woman would decide at the very beginning of the pregnancy what circumstances would lead to her decision for a cesarean section (Caitlin, 2005).This would eliminate the need to make the decision during a high stress time, since we can assume that such decision would cause stress, and at a time that the mother’s mental and emotional state is impacted by the high levels of hormones associated with pregnancy. I understand that being able to hold a woman to the advanced directives would be impossible, but a woman could elect to rely on the already issued directive and not add the trauma of making a decision to an already stressful time. This would also allow the person to discuss the eventualities with those whom she believes have a right to have a say in her life instead of just those that the laws say have a right to assist with her decision-making (next of kin, when the patient is incapacitated).Another article that drew my attention that I found in my literature review was a discussion about the ethical concerns some doctors have about making medical recommendations that are contrary to their own moral and ethical beliefs.â€Å"A growing number of doctors, nurses, and pharmacies are refusing to provide, refer, or even tell their patien ts about care options that they feel are not in keeping with their own personal religious beliefs,† stated Barbara Kavadias, Director of Field Services at the Religious Coalition and leader of the three-year project that created In Good Conscience. â€Å"Institutions are refusing to provide essential care, citing their religious commitments.† (Bioweek, 2007)This is a growing ethical trend in medical care that I have some major concerns with. Take, for instance, the case of my current patient. If she were (or is) being treated by a doctor who believes all life is sacred, he might be willing to risk the life of the mother in an effort to try to save the child. In this case, it is difficult to determine how a person with these moral concerns might treat the patient. Taking the child via c-section is probably the best for option to preserve the mother’s life. It may result in the immediate death of the fetus. Waiting and trying to abate the mother’s contractio ns may provide the child with a greater chance of survival, but also puts extra risk on the mother’s life. At that point, what are the criteria used by those with this moral outlook to determine the proper course of action?These questions are likely to grow in controversy as technology increases and the fetus is increasingly viable outside of the womb. The more that society becomes able to keep a child alive without the benefit of the mother, the more questions regarding the ethics of doing so or not doing so will grow in prominence. It is absolutely possible that with increasing medical technology and the ability to prolong life we will have additional debates regarding who gets to determine what lives are worth saving and what lives are lost.I believe that a trend toward making informed decisions is a good one and a move in the right direction, taking people away from having to make a decision in a crisis situation. I also think that it is worthwhile to discuss the role of the father in the decision-making process. Because of the trend toward increasing women’s rights and in an effort to prevent a return to the days of the complete male dominance, society appears to be moving away from the rights of a souse to have a say in decisions that affect them.For example, the birth of a child is an 18-year (minimum) commitment for men as well and in an effort to secure the rights of women, we have completely removed the father from the decision-making process. As a human, I believe that ultimate control of a person’s body should be his or her own, but it is also reasonable to believe that a spouse (or life partner) should have some say in the decision. Data mining has become a backbone for CRM activities. A. Content analysis for identifying the right customer The method of deciding on the units of analysis based on the direction of the study is known as content analysis. The conceptÂÂ   of computable and specifiable, categories are defined which help in grouping the processed units of data, quantify and analyze the samples. Content analysis is often used on various advertisements styles, in email communications and on social media. The figure below shows information flow from data collection to useable knowledge. Models and interconnections are developed after analysis which in turn help with decision support. This data when applied to a companies unstructured data is used to find the core (right) customer which in turn helps in planning efficient business strategies and providing appropriate customer service to different customers. B. Mining Customer behavioural changes In an ever changing business environment, trying to analyse customer behaviour is very helpful. Customer and transaction databases can be used to analyse customer behaviour by using data integration and transformation. As per the figure above, customer, product and transaction databases are used to analyse the customer behavioural variables: Recency, Frequency, Monetary (RFM). Recency represents most recent transaction time, frequency is the number of purchases during a certain period and monetary isÂÂ   the standard amount of expenditure. The attributes, frequency and monetary, are used to segment the customer into different categories namely: Uncertain, Frequent, Spender, Best. The Recency variable can be combined with the above study to develop a target market. Association rules are used for mining customer behaviour by analyzing the relationship of products purchased by a customer in retail stores. A classic application of association rules is the market basket analysis where products bought by a customer during a visit to the supermarket are analysed. This can be used to identify corelations between product purchase and customer profile represented by demographic variables. Customer behavioural data are most effective for generating predictive data which optimizes the CRM. The study of investigating change in customer behaviour is called change mining. Change mining looks at changing customer behaviour to develop some pointers which can be used to mathematically quantify the change in beahviour.ÂÂ   The output of all the above is analysed data which can be used to support efficient marketing. C. Learn and Usage model for Customer retention Loosing current customers to a rival company is termed as customer churn.ÂÂ   Finding better methods for customer retention is very crucial as acquiring new customers proves to be very expensive.This is two phase model: Learning and Usage. The learning phase constructs a churn model which tests and predicts the probablity of defection for a certain customer based on historic data. A policy model is also constructed which clusters the churners by grouping them based on notable attributes. These are then used for creating proper policies for individual groups. During the usage phase the churn model predicts if a certain customer would defect and when there are strong chances of defection, the policy model comes up with relevant policies to retain them. This method not only predicts churning but also helps in reducing it. The following figure shows the learner mode architecture: Fig3: Architecture for learning mode [5] During the learning mode the churn model learner is constructed using histiorical data of individual subscriber like loyalty history, deactivation data, payment history, usage patterns, etc. The churn model can be represented as a decision tree which is used to comment on the likelihood of a specific customer defecting based on their previous data. The policy model constructor is used to build retention strategies for potential churners. The policy model builds retention strategies in two steps, first step is to consult learned churn model to recognize the attributes which have strong relation to the churning and based on these attributes the churners are classified into diferent groups and labelled as per the most significant attribute. Second step is to analyse the significance of these attributes and reccomends policies for retaining the group of churners. In the usage phase the churner model is consulted to predict the chances of customer churn based on the customer data. If the churn probablity is more than 60% then it is considered that the customer has high chances of churning and so policy model proposes a policy response based on the attribute of the group to which the customer belongs for the purpose of retention. This journal reviews the literature concerned with data mining techniques and its applications in CRM. Considering the current competition in the market all organizations suffer from a lot of customer churn which results in huge losses as the cost for acquiring new customers is ten times more than that of retaining existing customers. Hence a lot of research has been made on customer retention. This is also evident from the number of reasearch papers submitted between the years 2000 2006 [1]. The area of customer retention definetly seems to be crucial and requires further research.
